OFLOXACIN 400mg

DOSAGE Adults: 200-400mg daily usually for 5-7 days depending on type & severity of infection. Doses greater than 400mg should be given in 2 divided doses. UTI: 200-800mg daily & in Gonorrhoea 400mg as single dose. A dose of 400mg daily or intermittently by mouth has been recommended by WHO, as a part of alternative multidrug therapy regimens for Leprosy.
Composition Pale yellowish-white to light yellowish white crystals or crystalline powder. Slightly soluble in water & in methyl alcohol. Exposure to strong sunlight & u.v.rays to be avoided.
Antimicrobial Action Ofloxacin is bactericidal & acts by inhibiting the A subunit of DNA gyrase which is essential in the reproduction of bacterial DNA. Its activity may be reduced in acidic media. Ofloxacin is more active than ciprofloxacin against chlamydia trachomatis, Mycobacterium leprae as well as M. Tuberculosis & some other mecobacterium spp. Synergistic activity against M.Laprae has been reported between Ofloxacin & Rifabutin.
Absorption Ofloxacin is rapidly & well absorbed from the G.I.Tract. Oral bio availability is almost 100% & a peak plasma concentration of 3 to 5 micrograms/mL is achieved in 1 to 2 hours after a dose of 400 mg orally. Absorption may be delayed by the presence of food, but the extent of absorption is not substantially affected. The plasma half life ranges from 4 to 7 hours; in renal impairment values of 15 to 60 hours have been reported. About 25% is bound to plasma proteins & is widely distributed in the body fluids, including the CSF, & tissue penetration is good, also appears in the bile. It is eliminated mainly by the kidneys. Only small amounts of Ofloxacin are removed by haemodialysis.
Indications Genito urinary, respiratory, Gastro-intestinal, Skin & soft tissue infections, Peritonitis, Gonorrhoea.
Contra- Indications Hypersensitivity to 4-quinolones, history of epilepsy.
Drug Interactions Antacids containing magnesium & aluminium reduce absorption. Sucralfate reduces absorption. Inhibits GABA receptor binding Probenecid decreases climination.
